The OnPointe approach to this Medicare program is to provide a home health care team that includes a transitional coach and an on-staff nurse practitioner—something not all home health agencies can provide—to support the patient in developing four important self-care management skills:
- Managing medications
- Scheduling and preparing for follow-up care
- Recognizing and responding to red flags that could indicate a worsening condition
- Taking ownership of a core set of personal health information
When done independently, these skills have proven to offer a patient a better quality of life through being able to take care of themselves and prevent future hospital visits. Until then, the transitional services are provided in a patient's home and/or via telephone where the nurse can identify if a patient is appropriate for palliative care, then meet with the patient and his or her family on the day of discharge from the hospital and as often as needed throughout their journey.
The nurse utilizes liaison tracking, risk level notation to determine an appropriate level of care and both timeline and skill level assessments for the patient. He or she also serves as the connection between the patient and our OnPointe At Home office, scheduling the patient’s visits and checking them in on the days of those visits. Patients are billed and validated within a 30-day process, completing an otherwise intricate set of steps needed to get adequate care and start feeling better.
