Hospice care at OnPointe At Home provides a comfortable, supportive level of care for patients and their families centered on compassion and respect during what we know is one of the most challenging times in life.
Our program includes pain and symptom management, respect for the patient’s and family’s wishes, dignity, support, peace, and respect for the fullness of life throughout the process, as well as both physical and emotional support for the family and friends. It’s an extremely difficult process for anyone, and OnPointe At Home’s network of care can help make it as pain-free as possible.
For people with a life-limiting illness of six months or less and who desire no further curative measures, such as chemo or radiation therapy, OnPointe At Home’s hospice care can help you and your family or loved ones through the referral process. We often describe ourselves as a “hospital without walls” because we employ a broad network of healthcare services from the beginning to the end of the journey. Our intimate knowledge of where you’ve been and where you want to go helps us provide the care you need right now. Simply, we know your journey and can walk it with you.
Whether in your home, a nursing facility or a hospital, depending on the circumstances of each individual patient, our hospice team includes your personal physician, the hospice medical director, nurse, home health aides, social workers, counselors and compassionate, trained volunteers who work together to provide the specific needs of each individual patient.
Our hospice services include:
- Medications related to the patient’s comfort needs or terminal diagnosis
- Medical equipment needed to maintain patient comfort and safety
- Routine care in your place of residence
- Respite care for caregivers
- Continuous RN care in the home in times of medical crisis
- Bereavement services and counseling
- On-call hospice support, 24 hours-a-day, seven days-a-week

The Professional Services provided by OnPointe At Home are part of the Medicare program called Transitional Care Management, a program created in 2013 to help patients manage their healthcare needs following a discharge from an Inpatient Acute Care Hospital, Skilled Nursing Facility, Long Term Care Hospital, Inpatient Rehabilitation Facility or Hospital, outpatient observation or partial hospitalization.
Home health care services provided by OnPointe At Home are ordered by your physician and provided by our team of skilled nursing professionals, therapists and/or trained attendants. Because we have over 100 years of experience across many levels of healthcare, we can provide the best quality care for each individual patient, no matter what those specific needs might be.
