Through regular and frequent visits with patients and their families to establish goals and provide both support for and open communication about the patient’s progress, they can make better decisions that often lead to a patient’s recovery and ultimately a better quality of life.
It’s important to know the difference between palliative care and hospice care. At OnPointe At Home, hospice care begins after treatment of the disease has stopped and when it is clear that the person is not going to survive the illness. Palliative Care begins at diagnosis and is employed at the same time as treatment to alleviate the pain and discomfort of the illness’s symptoms.
Because pain can inhibit participation in daily activities and thus make people appear to need more assistance than they would if their pain was under control, our nurses work closely with your primary care physician and referring specialists to provide the best possible palliative care. This includes developing a pain management program that works for you and your family, often combining long and short-acting medications to help you reach the level of comfort you desire.

The Professional Services provided by OnPointe At Home are part of the Medicare program called Transitional Care Management, a program created in 2013 to help patients manage their healthcare needs following a discharge from an Inpatient Acute Care Hospital, Skilled Nursing Facility, Long Term Care Hospital, Inpatient Rehabilitation Facility or Hospital, outpatient observation or partial hospitalization.
Home health care services provided by OnPointe At Home are ordered by your physician and provided by our team of skilled nursing professionals, therapists and/or trained attendants. Because we have over 100 years of experience across many levels of healthcare, we can provide the best quality care for each individual patient, no matter what those specific needs might be.
